2026-14: Mental Health and Substance Use Case Manager

We are seeking a compassionate and skilled Mental Health and Substance Use Case Manager to join our Saplings mental health team.
The Mental Health Substance Use Clinician provides outpatient and community services to adolescents age 12- 24 who are experiencing co-occurring substance use and mental health issues. The Mental Health Substance Use Clinician will work within an interdisciplinary team and will provide intake, assessment, treatment planning, and counselling to clients referred through the community.
Key Job Duties:
- Complete strengths and needs assessments, develop service plans, and support clients in reaching their goals.
- Provide counselling, outreach, progress monitoring, risk assessment, and transition planning for youth, young adults, and families.
- Coordinate referrals, transfers, telehealth access, and case management for specialized or out-of-community mental health and substance use services.
- Facilitate care planning with interdisciplinary teams, community providers, families, and natural supports.
- Identify service gaps and develop community training, groups, and programs that support substance use education, harm reduction, and local youth wellness.
- Collaborate with parents, schools, non-profits, private providers, and government services to ensure coordinated, client-centered support.
- Cleared criminal record and vulnerable sector check.
- Maintain valid driver's license and access to reliable vehicle (milage paid by employer).
Position Details
Hours: 35 hours per week
Employment Type: Permanent full-Time
Wage: This is a BCGEU unionized position, Addictions Counsellor at Grid 14P. ($40.03-$50.59/ hour, based on seniority or equivalent experience.) You will also get enrolled in our benefits program and Municipal Pension Plan after completion of your probation period.
Qualifications
Bachelor’s degree in Social Work, Psychology or a related field.
Minimum of 2 years clinical experience post degree.
Ability to treat and case manage complex substance use and mental health conditions that may encompass: concurrent disorders, multiple barriers, behavioural conditions, neurodevelopmental disorders.
Familiarity with local adolescent and adult mental health and substance use resources.
Knowledge and practical experience in working with the Mental Health Act, Child Protection Act and Infancy Act.
